Description: Chronic Disease Screening, Prevention, Management and Treatment Program for Diabetes, Asthma, Obesity and Hypertension available in all languages.
-The Chronic Disease program at La Maestra provides educational service to patients and their families suffering from a range of chronic illnesses including Asthma, Diabetes, Obesity and Hypertension.
-Weekly courses are offered about the diseases, how they can be prevented and self-managed. The program also offers access to medications at little or no cost to patients who are underinsured and would otherwise have to go without.
-The Health Educators offer advice about using treatment and testing equipment and how to apply for free medication.
-Participants' progress and condition are carefully case-managed in collaboration with their primary care provider as part of La Maestra's Circle of Care.
-Blind results are submitted to disease registries for analysis and further program modification and expansion.

Nutrition and Weight Management Program
Description: Weight Management and Disease Prevention

-Services are provided for overweight patients and their families as well as anyone interested in learning about healthy lifestyles and eating habits. The program is designed to provide culturally and linguistically appropriate food alternatives and advice.
-The curriculum for the five-week nutrition and weight management program is based on prevention of diabetes, cardiovascular disease, hypertension, obesity, and other weight-related health issues. Topics include:
Class 1: Program overview, introduction to food pyramid and recommended daily portions. Personal weight management plan established.
Class 2: Food portion sizes
Class 3: How to read food labels and calories
Class 4: Calculating BMI and physical activity
Class 5: Disease prevention, blood pressure, cholesterol, and other health education
-Each 60 minute session provides participants with tools to improve or better manage their weight; modify behavior, diet and lifestyle; and increase physical activity to prevent or delay the onset of a chronic disease.
-In addition, there are 3 and 6 month follow-ups to monitor participants' progress and assess their lifestyle changes.
