Description: Diabetes Prevention, Screening, Management and Treatment Program

Languages: English and Spanish

 The Chronic Disease program at La Maestra provides educational service to patients and their families suffering from a range of chronic illnesses including Asthma, Diabetes and Obesity. Weekly courses are offered about the diseases, how they can be prevented and self-managed. The programs also offers access to medications at little or no cost to patients who are underinsured and would otherwise have to go without. The Health Educators offer advise about using treatment and testing equipment and about applying for free medication. Participants’ progress and condition is carefully case managed in conjunction with their primary care provider. Blind results are submitted to disease registries for analysis and further program modification and expansion.

 

Funding / Special Programs

 This program is funded through a grant through the Bureau of Primary Care’s  Healthy Communities Access Program (HCAP)

 

 

 

Diabetes Classes

 

 Hours:

English:        Mondays     11:30-1:30pm

 Spanish:       Fridays     10am – 12pm

 

 General Diabetes Education and Assistance

                      Monday – Friday     9am – 6pm
