Job Title: Data Entry Specialist

Summary
Hourly rate $22 – $26
The Data Entry Specialist will ensure that all data collection, data entry, data management and tracking are fulfilled for the Comprehensive Community Behavioral Health and Wellness Clinic (CCBHWC). The successful candidate will have a sincere interest in customer service and speaking with patients to obtain behavioral health, substance use, and quality of life metrics. The candidate will complete data collection in a timely and patient-centered manner. The Data Entry Specialist will have excellent communication skills with staff and patients, pay close attention to detail, and demonstrate time management skills to handle a variety of activities and tasks.
Responsibilities
- Conducts questionnaire covering behavioral health, substance use, and wellness with patients to obtain data to enhance the program and patient care.
- Conducts data entry to support program evaluation processes, including screening measures, federal GPRA/NOMs data tools, and other outcome assessments.
- Reviews data for thoroughness and quality, and assures that it is in accordance with the established policy and funder requirements.
- Implements a sustainable and user-friendly data tracking process and documents procedure for data collection.
- Maintains confidential program and client records in tracking system and EHR.
- Maintains expertise in the details of the GPRA data tool, quickly learns updates that are made to the tool, and ensures everyone using the tool is trained on updates.
- Represents the department in a professional and congenial manner in person, in telephone conversations, and in written correspondence.
- Documents interactions with patients in EHR.
- Complies with all patient privacy regulations.
- Obtains basic physical health indicators and vital signs from patient or medical record.
- Attends grant evaluation and data quality review meetings in addition to other relevant dept meetings.
- Prepares communications, such as emails, reports and other correspondence.
- Demonstrates effective communication, prompt problem solving skills, and proactive support to co-workers.
- Provides exceptional customer service.
- Discreetly handles confidential information.
Patient Centered Medical Home Essential Responsibilities (Principals)
- Collaborate with individual patients, their personal physicians, and when appropriate the patient’s family to ensure accessible, comprehensive, compassionate, and culturally competent care
- Facilitate care by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.
- Ensure all decisions respect patients’ wants, needs, and preferences and that patients have the education and support they require to make decisions and participate in their own care.
- Continually strive to attend to each patient’s “whole person,” in the context of the patient’s personal and medical history and life circumstances, rather than focusing on a particular disease, organ, or system
Job Requirements
Education: High School Diploma or GED Equivalent Required
Experience: A minimum of two years of data collection, administrative, or customer service experience required. Requires experience with Microsoft Excel, Word, PowerPoint and Outlook. Experience working with children and adults with mental and behavioral health challenges and/or substance use preferred.
Verbal and Written Skills to perform the job: Bilingual (English/Spanish) preferred. Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills.
Technical knowledge and skills required to perform the job: Excellent verbal, written and interpersonal communication skills and the demonstrated ability to work with diverse individuals and groups. Demonstrate high levels of self-initiation and direction. Knowledge of communities we serve. Knowledge and experience working in medical settings and interacting collaboratively with medical teams. Skill in analyzing situations and making timely decisions.
