Job Description

Salary Range: $104,000.00-$124,800.00- DOE

Summary

The Marriage and Family Therapist is responsible for providing a wide range of culturally and linguistically appropriate mental health and psychosocial services. The MFT will provide direct practice mental health services such as clinical assessments and evaluations, treatment planning, individual/ group/ family/ couple therapy, crisis intervention, clinical case management, and internal and external care coordination.

Responsibilities (including but not limited to):

  • Conduct biopsychosocial evaluations on patients and/or families to assess the extent to which health-related, psychological, social and spiritual factors may be contributing to presenting problems.
  • Utilize validated tools, methods and approaches to assess, diagnose, and inform treatment.
  • Prepare individualized care plans (IAP) to address the unique biopsychosocial needs of patients, in objective and measurable ways, within 30 days of initial patient contact.
  • Provide ongoing treatment services, monitor progress of treatment, refer for additional services, re-evaluate, and terminate services as necessary.
  • Facilitate individual, group, and couple therapy for patients and/or families using evidence-based practices and within the providers scope of training and practice.
  • Meet with family members and significant others to gain collateral and enlist their support in carrying out the patient’s treatment.
  • Provide crisis evaluation, clinical case management, and other supportive services. Refer, link and facilitate access to appropriate community entities.
  • Provide crisis intervention, emergency referrals, and urgent care coordination as needed.
  • Work collaboratively with other treatment team members (i.e. psychiatrists, primary care physicians, case managers, etc.) to coordinate and provide integrated physical health and behavioral health services to patients.
  • Use La Maestra and other validated tools to complete and document intake assessments, reassessments, IAPs, progress notes and discharge summaries. Track and ensure all assessments are up to date.
  • Generally, complete documentation within 2 business days of services rendered, with consideration for expedited or delayed reporting factors.
  • Assist with maintaining up to date medical records to include contact information, demographics and funding information. Report accurately the nature and length of all services rendered.
  • Remain familiarized and up to date with current behavioral health methods and practices.
  • Be knowledgeable about and uphold confidentiality and privacy practices.
  • Maintain productivity standards.
  • Contribute to a warm, welcoming and professional environment for patients, coworkers, partner agencies and the general public.

Additional Duties and Responsibilities

1.   Attend staff meetings.

2.   Performs other duties as assigned.

3.   Complete assigned trainings.

4.   Maintain active licensure in the state of CA.

Patient Centered Medical Home Essential Responsibilities (Principals)

  • Collaborate with individual patients, other care providers, and when appropriate the patient’s family to ensure accessible, comprehensive, compassionate, and culturally competent care.
  • Facilitate services by registries, information technology, electronic health records, health information exchanges and other means, to ensure culturally and linguistically competent care.
  • Ensure all decisions consider patients’ needs and preferences, and that patients are fully informed to participate in their own care and make care decisions.
  • View patients from a “whole person” perspective, considering each unique patient’s entire biopsychosocial and spiritual history, rather than focusing on a particular disease, organ, or system in isolation.

Job Requirements

Education: Master’s Degree in Marriage and Family Therapy. Degree must have been conferred by a school, college, or university accredited by the Western Association of Schools and Colleges.

Certification: Active Marriage and Family Therapist license in the state of California.

Experience: Experience working in a community health or mental health setting.

Verbal and Written Skills to perform the job: Bilingual preferred. Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills.

Technical knowledge and skills required to perform the job: Excellent verbal, written and interpersonal communication skills and the demonstrated ability to work with diverse individuals and groups. Demonstrate high levels of self-initiation and direction. Knowledge of communities we serve. Knowledge and experience working in medical settings and interacting collaboratively with medical teams. Skill in analyzing situations and making timely decisions. Electronic Health Recording experience preferred.
