Salary Range: $22.50-$29.00 Per Hour (DOE)

Job Description

Summary:

This position provides our most vulnerable population with support services with accessing medical and dental health services, assistance with attaining shelter, transitional housing, permanent housing, advocate for recover, case management and education services.

Responsibilities (are not limited to):

  • Provides community outreach and early intervention activities by supplying information about La Maestra’s Circle of Care network and community agencies to assist clients of additional programs/services availability.
  • Provide assistance with registering new patients, schedule medical appointments, recall client’s appointments to ensure appointment is kept.
  • Provide enrollment assistance with MediCal, CalFresh, Financial Literacy, Identification Card Waiver form, Birth Certificate Waiver form, Social Security Card forms.
  • Assist with preparing events, hygiene kits, information packets, stock and shelf items in storage areas.
  • Maintains a caseload as prescribed by the Community Engagement Manager; provide support and advocacy to connect clients to substance abuse treatment programs.
  • Works directly with the participant to provide orientation to the resources and programs related to La Maestra’s treatment program and the concept of sobriety.
  • Gathers information for assessment and access points through La Maestra’s Circle of Care services; Conducts intake interviews and completes comprehensive individual assessment of participant’s financial status for housing placement programs and level of social functioning.
  • Monitors and assists participants needs and evaluates the progress of the individual participants according to the scope of needs requested.
  • Interacts with court and law enforcement officials as a team member and on a professional level while maintaining appropriate boundaries.
  • Provides crisis intervention and refers participants to community health, HIV and TB clinics as necessary.
  • Provides input to develop and improve program servicing and delivery.
  • Utilize motivational interview skills to assist clients with establishing daily goals, attentive behavior, ensure clients are informed of inappropriate behavior, acting as a role model and encouraging team building feedback and problem solving with clients.
  • Provide support other staff in dealing with problematic participant’s behavior.
  • Designates new target goals for participants consistent with overall goals.
  • Assists in community placement of the participants, aids in the location of needed services, provides personal support.
  • Maintain effective communications with supervisor, coworkers, with status updates on clients, reports, events, and whereabouts while on-duty.
  • Participate in scheduled Homeless Outreach activities coordinated by the Community Engagement Manager.

Job Requirements

Education: High School Diploma or GED

Verbal and Written Skills to perform the job: Bilingual (English/Spanish). Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills.

Technical knowledge and skills required to perform the job: Excellent verbal, written and interpersonal communication skills and the demonstrated ability to work with diverse individuals and groups. Demonstrate high levels of self-initiation and direction. Knowledge of communities we serve. Knowledge and experience working in medical settings and interacting collaboratively with medical teams. Skill in analyzing situations and making timely decisions.

Physical and Mental Requirements: Work involves sitting, talking, hearing, using hands to handle, feel or operate objects, tools, or controls, and reaching with hands and arms.
