La Maestra Diabetes Clinic

La Maestra’s patients have the option to attend either in-person or virtual DSMT program sessions, which cover topics such as signs and symptoms of uncontrolled diabetes; understanding HbA1c and blood glucose metrics, blood pressure both diastolic and systolic, and cholesterol numbers; healthy eating; managing stress; physical activity; medication safety; and preventing complications from uncontrolled diabetes.

Diabetes Clinic and Care Team staff are part of a multi-disciplinary effort to improve the quality of care for patients with diabetes and prediabetes. The program is currently run by a Family Nurse Practitioner (FNP) with a current Certified Diabetes Educator (CDE) credential. Other members of the team include an endocrinologist, primary care providers, health educators, and case managers. Staff members are bilingual and are therefore able to deliver high-quality, culturally and linguistically competent diabetes care, regardless of ability to pay. La Maestra hosts weekly diabetes workshops at clinic sites in City Heights, El Cajon, and National City. The schedule is as follows:
