Job Title: Health Education Case Manager
Reports To: Director of Health Education Services
Job Status: Exempt Non-Exempt
Hourly Salary: $25-$31
Summary
The Health Education Case Manager is an essential staff member in the Health Education Department. The case manager will be responsible to ensure that all La Maestra patients not only access our Health Education Programs, but also maximize their educational experience. The case manager will communicate and explain the benefits of our various services to the patients and ensure that they complete the programs in their entirety, when appropriate. The case manager will be responsible to communicate with the Health Education Team to ensure that there is clear communication on a daily basis of the educational needs and expectations for our patients. Additionally, the case manager will act as a support person that ensures patients are connected to the various departments within the organization as well as our community partners.
Responsibilities (are not limited to):
Track all patients that could benefit from an educational program through our various platform (i2i tracks, NextGen, etc.) reports
Track the education received by each patient through our various platform reports
Ensure that all patients understand and agree to complete the educational programs available to them
Call patients to schedule pending Health Education appointments and ensure that they are connected to the necessary La Maestra Departments and/or partnering organizations within the community
Ensure that patients make it to their Health Education appointment by scheduling transportation and/or interpretation assistance, if needed
Daily communication to the Health Education Team and supporting departments to ensure that all understand our patient’s health education plan
Review the Health Educator’s Schedules to ensure that the department is maximizing schedule times and to confirm accurate scheduling
Additional Duties and Responsibilities
- Attend and provide updates at the department monthly meetings.
- Attend and provide updates at the daily department huddles.
- Performs other duties as assigned.
Patient Centered Medical Home Essential Responsibilities (Principals)
Collaborate with individual patients, their personal physicians, and when appropriate the patient’s family to ensure accessible, comprehensive, compassionate, and culturally competent care
Facilitate care by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.
Ensure all decisions respect patients’ wants, needs, and preferences and that patients have the education and support they require to make decisions and participate in their own care.
Continually strive to attend to each patient’s “whole person,” in the context of the
patient’s personal and medical history and life circumstances, rather than focusing on a
particular disease, organ, or system.
Job Requirements
Education: High School Diploma or GED
Certification required: None necessary
Experience: Customer service, at least 2 years preferred
Verbal and Written Skills to perform the job: Bilingual (English/Spanish). Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills. Technical knowledge and skills required to perform the job: Excellent verbal, written and interpersonal communication skills and the demonstrated ability to work with diverse individuals and groups. Demonstrate high levels of self-initiation and direction. Knowledge of communities we serve. Knowledge and experience working in medical settings and interacting collaboratively with medical teams. Skill in analyzing situations and making timely decisions. Physical and Mental Requirements: Work involves sitting, talking, hearing, using hands to handle, feel or operate objects, tools, or controls, and reaching with hands and arms.
