Specialty Clinic Family Nurse Practitioner

Summary

 

Under the supervision of the Medical Director of Specialty Clinics, the Family Nurse Practitioner is responsible for helping patients with all aspects of care, including diagnosis (physical and psycho-social), treatments, and consultations.  This role involves working closely with La Maestra hepatologist, endocrinologist, and geriatrician to enhance patient care, coordinate treatment plans, and support integrated management of complex metabolic and liver-related conditions.

Responsibilities

  • Facilitate collaborative care between liver specialists, endocrinologist, and geriatrician, ensuring seamless communication and comprehensive treatment strategies
  • Participate in multidisciplinary case discussions
  • Coordinate diagnostic evaluations, follow-up care, and treatment plans for patients with fatty liver disease, diabetes, metabolic syndrome and other related disorders
  • Provides independent direct advanced practice nursing care, performing physical examinations, ordering diagnostic studies, evaluates results and prescribes treatment as appropriate
  • Examines and prescribes treatment for clinic patients, assists in activities for the detection and control of communicable diseases, consults with private physicians on diagnoses involving diseases; interprets laboratory data; renders professional services in school health, birth control, maternal and child health, venereal diseases, immunization and TB finding programs, etc.
  • Obtain, compile and record patient medical data, including health history, progress notes and results of physical examination
  • Administer or order diagnostic tests, such as x-rays, electrocardiogram, and laboratory tests
  • Prescribe therapy or medication with physician approval if applicable
  • Order appropriate treatments, medications and diagnostic studies
  • Respond to inquiries from referring physicians, hospital staff, and patients regarding the diagnosis and treatment of conditions as well as eligibility for research protocols
  • Prepares reports as necessary
  • Assume responsibility and accountability for the continuity of health care regardless of the presence or absence of disease CCR 1480 (b). The family nurse practitioner will be the only health professional to see the patient, and in the process, will employ a combination of nursing and medical functions approved by standardized procedures.
  • Performing physical and mental assessments, disease prevention and restorative measures, performance of skin tests and immunization techniques, and withdrawals of blood, as well as authority to initiate emergency procedures
  • Refers patients to other medical specialties as necessary. Provides education and advice to patients and their families
  • Maintain effective communication and working relationships with other employees and patients
  • Employees will be required to follow any other job-related instructions and perform other job- related duties requested by their supervisor

  • Collaborate with individual patients, their personal physicians, and when appropriate the patient’s family to ensure accessible, comprehensive, compassionate, and culturally competent care
  • Facilitate care by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.
  • Ensure all decisions respect patients’ wants, needs, and preferences and that patients have the education and support they require to make decisions and participate in their own care.
  • Continually strive to attend to each patients “whole person,” in the context of the patient’s personal and medical history and life circumstances, rather than focusing on a particular disease, organ, or system

Job Requirements

Education:  Master of Science Degree in Nursing from an accredited institution

Certification required:  Valid California Board of Registered Nursing Nurse Practitioner License, current active DEA License, and current active NP Furnishing Number

 

Experience: 1-2 years’ experience as a practicing Family Nurse Practitioner working with EHR systems preferred.

 

Verbal and Written Skills to perform the job:  Bilingual preferred. Familiarity with basic word processing, spreadsheet, and database applications.  Accurate keyboarding skills.

Technical knowledge and skills required to perform the job:  Posses and exercise the degree of learning, skill, and care ordinarily possessed and exercised by a member of the appropriate discipline in clinical practice. Excellent verbal, written and interpersonal communication skills and the demonstrated ability to work with diverse individuals and groups.  Demonstrate high levels of self-initiation and direction. Knowledge of communities we serve. Knowledge and experience working in medical settings and interacting collaboratively with medical teams.  Skill in analyzing situations and making timely decisions.
