Summary:

As a Prenatal and Family Medicine Case Manager is responsible for providing comprehensive case management for patients. Receives referral reports from physicians and medical assistants, calls and responds to patients’ needs in person or by telephone. The Prenatal Case Manager performs the designated case management within the clinic, assists and carries out the physician’s orders and executes vital clinical tasks in a professional and efficient manner.

Responsibilities:

  • Responsible for assessment, case plan development and advocacy.
  • Interviews patients to ascertain nature of referral visit. Records information on standard forms and logs.
  • Communicates with physicians or medical offices to which the patient us being referred for medical care. Routes form to appropriate physician or office for action.
  • Compiles referrals for records, by category, department office, and disposition.
  • Notifies supervisors of patterns of poor provision of service.
  • Maintains up to date reference materials and files.
  • Communicates with patients to make appointments.
  • Meet minimum requirement of 20 referrals per day.
  • Maintain effective communication and working relationships with other employees and patients.
  • Maintains spreadsheets and case management work flows to proactively manage OB/GYN patients so they are compliant with their care.
  • Updates providers appropriately when needed.
  • Reports noncompliant patients to medical director after a risk of discharge letter is sent.
  • Documents all interactions with patients and creates addendum documents when a status update is learned about a patient to communicate with the rest of the staff through Nextgen.
  • Communicates to patients and associated staff caring for the patient using appropriate methods in a timely manner.
  • Documents work flows as they are created and updated.
  • Uses the case management software and other computer and paper methods appropriately to case manage.
  • Improves the connection between the OB/GYN department and other departments including outside entities caring for our OB and postpartum patients.
  • Works in a methodical manner to keep track and manage all patients under Prenatal Case Management.
  • Other duties as assigned.

Clerical Responsibilities:

  • Maintains a clean, safe, and functional work station.
  • Maintains adequate quantities of patient supplies and forms.
  • Calls and/or sends a letter for all no show patients and records the phone call within NextGen.
  • Obtains patient’s signatures on permission/consent forms.
  • Generate referral note in NextGen when referral is ordered by a provider
  • Follows-up on the patient’s referral appointment, if applicable.
  • May rotate or be assigned to other areas in the clinic as needed at the discretion of the clinic coordinator/manager.
  • Attends and participate in clinic staff meetings.
  • Calls no show appointments and re-schedules the appointment and documents on NextGen
  • Case manages patients.
  • Attend staff meetings.

Job Requirements

Education: High School Diploma or GED.

Certification required: Medical Assistant Certificate, Valid CPR card.

Experience: 1-2 years experience in physician office or hospital setting as a medical assistant, patient services representative, or similar experience. Strong track record of identifying barriers to care for patients, and helping them overcome such barriers.

Verbal and Written Skills to perform the job: Bilingual (English/Spanish) preferred. Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills.

Technical knowledge and skills required to perform the job: Excellent verbal, written and interpersonal communication skills and the demonstrated ability to work with diverse individuals and groups. Demonstrate high levels of self-initiation and direction. Knowledge of communities we serve. Knowledge and experience working in medical settings and interacting collaboratively with medical teams. Skill in analyzing situations and making timely decisions.
