Job Title:    Enhanced Care Management

Pay: Salary range- $23.00- $28.00
Location: City Heights
Hours of Work: TBD
Status/ Hours: Full Time/ Non-Exempt

Responsibilities:
• Collaborate with multidisciplinary care team in coordinating a wide array of outreach, health care, case management, and member engagement functions in recruiting, supporting and connecting individuals to medical and social services.
• Facilitate enrollment of members in appropriate medical and supportive services and programs based on the patient s health status, needs, preference, and goals.
• Provide organizational and administrative support for ECM and CS activities to help achieve desired program goals.
• Completes member assessments into ECM and CS Health Risk Assessment and Individualized Care Plan review process towards initiating cases, referral documentation, referral to other related programs as applicable.
• Coordinate with external entities providing member with some level of care
• Coordinate with primary care providers, specialists, behavioral health, community based long term services and supports needs and oral health providers involved in the care of the member.
• Promote Self-Management education, healthy behavior changes motivation and medication and appts compliance
• Scheduling appointments and providing appointment reminders.
• Coordinating transportation and possible accompaniment to critical appointments.
•Identify and help to overcome barriers to adherence to care plan; Promote, coordinate and tailor services to meet identified needs.

Job Requirements:

  • Bachelor’s Degree in public health, human, social welfare or related field; or equivalent experience, training, or coursework.
  • Minimum one year of progressively responsible and direct work experience working with the essential duties and responsibilities described above.
  • Demonstrated experience leading/managing projects, initiatives, and/or leading or directing the work of others.
  • Knowledgeable in the field of home and community-based services and community resource networks and a particular interest in working to address health disparities and addressing the needs of low-income communities.
  • Familiarity of SDOH that impact San Diego County and community members.
  • Ability to think creatively and work strategically, to help develop and implement innovative solutions yielding measurable results to the organization.
  • Ability to understand, interpret, and apply applicable rules and regulations, and establish and evaluate priorities.

  • Advanced working knowledge and proficient with Microsoft Suite applications, and the ability to operate all applicable software

Experience:

Demonstrate proficiency with computers and software including Microsoft Office Suite and electronic health records systems.

Work just as well independently as they do on a team.

Preferred: Significant knowledge of physician payment methodologies.

Exercise mature judgement, and are highly motivated, self-starting and proactive. Are excellent at communicating, whether in writing or verbally.

Verbal and Written Skills to perform the job:
Bilingual (English/Spanish) or (English/Arabic) with demonstrated language proficiency, oral and written (preferred).

Technical knowledge and skills required to perform the job: Familiarity with basic word processing, spreadsheet, and database applications with accurate keyboarding skills. Excellent verbal, written and interpersonal communication skills and the demonstrated ability to work with diverse individuals and groups. Demonstrate high levels of self-initiation and direction. Knowledge of working effectively with individuals from a diverse range of cultures and socioeconomic backgrounds we serve. Knowledge and experience working in community health settings and interacting collaboratively with inter disciplinary teams.
